Chaos mode Successfully complete all stories with an "S" rank to unlock the Chaos difficulty setting.
Extra item slots Purchase all the upgrades at the Buy/Ability menu for the stat in the indicated level you are in (for example, attack, defense, life, Musou) to unlock another item slot:
